This form enables our customers to submit a service repair order (SRO) / return merchandise authorization (RMA) request online.
Alternatively you have the option to print this SRO Form and fax it to us at (972) 423-1540.

Products to be returned for repair must have a Service Repair Order (SRO) number prior to shipment. The SRO number should be noted on the box and on the paperwork that accompanies the returned product. The SRO number can be obtained by contacting Luminator Repair Service at (972) 516-3120. The return of any product shall be at the expense of the Buyer. Product returned to Luminator must be properly packaged. Luminator will not be responsible for damage to a product during transit or packages lost in transit. Material should be shipped to:

All submissions received after 4:00 p.m. CST Monday - Friday, will be process the following business day. You will be emailed your Service Repair Order (SRO) number and instructions on returning your item(s) for service.
